Take a new health services survey

The UNT Mental Sciences Institute is helping Collin County respond to your needs. We are surveying county residents to find out what mental health and substance abuse services are needed here. The survey will only take about 10 minutes to complete, your answers are confidential, but the benefits of your feedback are great.

If you know anyone without internet access, paper survey forms will be available very soon at locations throughout the county. Stay tuned for a listing of paper survey distribution points.
