​​​​​​​​​Indigent Health Care Programs: This program provides health care services to qualified, adult indigent residents of Collin County. Proof of income and resources are required to determine eligibility. Please see the Links and Resources at the bottom of this page for eligibility forms.

If you would prefer to mail your application or submit the completed application in-person, please use the Indigent Healthcare Program Application PDF. Once a completed application is received along with all supporting documentation, a decision regarding your eligibility​ will be made within 14 business days. If you have any questions, you may contact our office during normal business hours.
