Our Services

  • Additional Health Care Assistance

    Collin County helps fund non-profit health care providers and clinics throughout the area that offer various health care needs from visual screenings for kids to prescription assistance to medical help for seniors, and more. Eligible residents may apply for these benefits at the inpidual clinic locations listed.

  • Collin County Indigent Program

    This program provides health care services to qualified indigent citizens of Collin County. Proof of income and resources are required to determine eligibility. Contact us at (972) 548-4702 for more information.

  • Epidemiology & Disease Reporting

    Investigating and studying diseases and health conditions in Collin County through information reported to county physicians from hospitals, schools, the public, and other local, regional, and state health departments. Also, see Immunizations

    Disease prevention is the key to public health. We administer immunizations to children and adults.

  • Immunizations

    The Immunizations Clinic provides vaccines to eligible children and adults​.

  • Medical Record Request
  • Individuals can receive a copy of medical records by filling out a request form, which is available in English and Spanish, and following the directions below.

    1. The forms can be:
      • a. Dropped off in-person at 825 N. McDonald Street, Suite 130, McKinney, TX 75069

        b. Mailed to 825 N. McDonald Street, Suite 130, McKinney, TX 75069. Please notate "Attention: Medical Records Department""".

        c. Faxed to (972) 548-4441

    2. Photo identification for the person that the medical record belongs to or their legal guardian is required to be provided prior to the medical record request being completed. Supporting legal documents may be required in circumstances involving legal name changes, guardianship, adoption, etc. If the identity documents are not provided in-person, they may be included with the mailed or faxed Medical Record Request Form.

    3. Fees associated with records requests can be found in the Collin County Health Care Services Clinic Fee Schedule.

  • Perinatal Hepatitis B Prevention Program

    Ensuring mothers, pregnant women and their sexual partners can get crucial testing and the Hepatitis B vaccine series for preventive measures.

  • Primary Care Services (NextCare Program)

    This program is a partnership between Collin County’s Health Care Services and NextCare Medical Centers. This program is an option for families who earn less than 100% of the federal poverty guidelines and need of primary or urgent care services.

  • Public Health Emergency Preparedness

    The department of Public Health Emergency Preparedness works to prepare our community for natural and man-made disasters. Through the development of plans, training, and community involvement we make Collin County a safer place to live.

  • STD/HIV Clinic

    Our clinic provides comprehensive services for the screening, counseling, and treatment of sexually transmitted diseases for qualified residents of Collin County.

  • Substance Abuse Program

    Collin County Substance Abuse Program shall provide professional alcohol and drug prevention and intervention services to all Collin County residents. These services shall include assessments, referrals, information, education, and all other related services.

  • Tuberculosis Clinic

    Testing and X-ray referral services to patients who have been exposed or are at-risk of being exposed to the TB germ. We also offer treatment services to persons who have TB and need medication.

  • WIC: Women, Infants & Children

    WIC is a special supplemental food program that provides nutrition education and food vouchers for qualifying children and infants, as well as pregnant, breastfeeding, and postpartum women.
