Please Note: Death records are typically available approximately two weeks after date of death.

Death records are considered confidential and may only be issued to qualified applicants per Texas Administrative Code 181.28.

Qualified applicants are:

  • Parent
  • Grandparent
  • Sibling
  • Adult Child
  • Spouse
  • Informant named on Birth Certificate

Anyone other than a qualified application seeking a death record must provide a signed authorization statement naming the individual who is permitted to obtain the certificate.  The authorization must include a copy of the qualified applicant's and the authorized representative's government issued photo identification.

Required information to obtain a death certificate:

  • Full name as it appears on death record
  • Date and County of Death
  • Parent's names
  • Mother's maiden name
  • Relationship to deceased
  • Mailing address
  • Purpose of record

If requesting by mail, please include a notarized application, copy of valid government issued identification and appropriate fees.
